If you haven't seen one yet, here is the new 2018 High-Tech prescription form (right-clik and select "View Image" for a bigger picture on desktops/laptops).

You'll be glad to see that the Lifetime therapy box is finally gone. What a relief!

It also states clearly right next to the date that it's only valid for a maximum of 6 months, which is also welcome. At least now the consultants will be well aware of the need to sort out new prescriptions every 6 months, so hopefully that'll mean less headaches for us in the pharmacy.
