URGENT - Patient Level Recall of Seven Batches of Viridal Duo

Please see attached recall letters and fax-back form, relating to a patient level recall of seven batches of Viridal Duo, PA 891/15/1 and PA 891/15/3. The batches include authorised product and exempt supply. Please also see attached Dear Doctor letter. Pharmacies which have received affected product in the relevant time period will receive a hard copy letter in the post. Please read the instructions carefully, to perform the requisite actions.
